Viewing Social History

You can view Social History (SHxUI.dll) as a component in the Chart Review.

 One of the following options is displayed in the Social History section:

  • No Social History for This PatientThis option is displayed when a patient has not been assessed in any of the social history categories using the Quick Assessment or any answers documented in any of the social history categories. This cell is nonactionable when this option is displayed.
  • Unable to Obtain: This option is displayed when the social history cannot be captured. This cell is nonactionable when this option is displayed.
  • [Social History Category]: This option is displayed for each social history category that is available for the patient.

Viewing Social History Details

The Social History split view contains a list of the patient's social history categories that are documented in the left pane.

Note

The first category listed is selected in the left pane by default when the view opens and the corresponding detail view is displayed in the right pane.

To view details for a category, select the category and view the information in the right pane.

The left pane contains the following information:

  • Category Name: The name of the category is displayed.

  • Category Details: The beginning of the list of details is displayed using the following hierarchy:

    • If the category questions have been documented, the defined responses and response labels are displayed.

    • If the category questions have NOT been documented, but the quick assessment has been documented, the assessment value is displayed.
    • If both the category questions and the quick assessment have been documented, the responses and response labels are displayed. The quick assessment is available in the detailed view.

The right pane contains the following details when known:

  • Category Name: The name of the social history category is displayed.
  • Quick Assessment: The quick assessment is displayed if it has been documented.
  • Questions and Responses: The questions and responses that have been documented for the social history category are displayed in the detail view. The sequence of the questions recognizes the build defined in the Social History tool.
  • Comments: Comments associated with the social history category are displayed in reverse-chronological order. The newest comments are displayed at the top of the list. In addition, the section displays the following identifying information:

    • The person who documented the comment.
    • The date and time of the comment entry.
    • The date and time in the device's regional format if the comment was entered on a date before the current date.

To navigate back to the Chart Review view, select Back.
